Winner Release and Liability/Publicity Release Form
My signature below indicates that I understand the following:
I have won the 2020 ‘Signature Scottsdale Getaway’ from Experience Scottsdale. The prize package is as follows:
- Round-trip airfare for two to Scottsdale via Phoenix Sky Harbor ($600 total maximum)
- Three-night stay for two at The Phoenician, a Luxury Collection Resort (restrictions/blackout dates apply)
- Dinner for two at Fax Ox
- Hot Air Balloon Ride for two from Rainbow Ryders
- Prize is not transferable or redeemable for cash.
- Winner and guest must be 21 or older, or winner must be guest's parent or legal guardian.
- Total approximate retail value of entire package is $3,050.00
- Prize must be redeemed by May 1, 2021. Blackout dates may apply.
- All taxes, gratuities, incidentals and other expenses not specified herein are the sole responsibility of the winner.
- Experience Scottsdale is required by the Internal Revenue Service to issue a 1099-MISC to the winner and to submit a copy to the IRS. I understand that I am responsible for any and all taxes associated with the receipt of this prize.
- Experience Scottsdale, their respective parents, subsidiaries, affiliates, and respective directors, officers, employees and agents are not liable for any injuries, losses or damages of any kind caused by any prize or resulting from acceptance, possession or use/misuse of any prize.
- I release Experience Scottsdale of its affiliates and agencies from any and all liabilities, and I agree to accept this prize in accordance with the terms set forth.
- I have read and accept Experience Scottsdale’s Privacy Policy.
